چکیده
The efficacy of chloramphenicol: in the treatment of acute typhoid fever is well established.84' 9 l0'1 18 However, treatment of the typhoid carrier state with this antibiotic, for varying periods of time and at dosage levels generally accepted as high, has been unsatisfactory.2 Untoward reactions have also been reported,' 2,6 of which anorexia, nausea, and a bitter taste in the mouth were the most frequent manifestations, although occasionally glossitis and cheilosis occurred in persons under prolonged treatment with large doses of the antibiotic. This communication presents the results of a quantitative study of the carrier state and the influence of chloramphenicol upon it in two chronic typhoid carriers. In addition, a description of the development and treatment of untoward oral manifestations similar to those previously described is included. Both subjects of this investigation had a history of acute typhoid fever (patient E. G., 24 years before, and patient R. L., 48 years before this study) and both had been known typhoid carriers for eight years. The following are pertinent data concerning these patients:

Chloramphenicol inhibits protein synthesis in bacteria and is usually bacteriostatic but is bactericidal against Haemophilus influenzae, Streptococcus pneumoniae, and Neisseria meningitis. Chloramphenicol penetrates all body tissues well. The cerebrospinal fluid concentration averages 60% of the serum level, while brain levels are 9 times higher because of high lipid solubility of this drug. Ch...

THE change in the clinical course of typhoid fever due to treatment with chloramphenicol has been one of the more dramatic triumphs of the antibiotic age. A prolonged fever has been converted into a short sharp febrile illness, so that the patient suffering from the disease, instead of being dangerously ill for several weeks, may well be sitting up in bed after three or four days on chloramphen...

Prior to the use of chloramphenicol in the treatment of patients with typhoid fever the mortality rate in this disease was almost 10 per cent and approximately one-fourth of the deaths were attributable to the toxic-febrile state (1). Chloramphenicol has been shown to control the bacteriologic manifestations of typhoid fever within a matter of hours but the toxemia has responded more slowly to ...
